Upsher-Smith expands nationwide product recall

Upsher-Smith Laboratories is voluntarily expanding its previously announced recall of Jantoven (warfarin sodium) 3mg tablets to include additional products that were packaged on the same packaging line between May 17, 2010 and November 17, 2010. The recall was first initiated as a precautionary measure after a single bottle labeled as Jantoven Warfarin Sodium, USP, 3 mg Tablets was found by a retail pharmacy to contain tablets at a higher 10mg-strength. The expanded recall now includes amantadine 100mg tablets; amlodipine 5mg tablets; baclofen 10mg tablets; bethanecol 5mg, 10mg, and 25mg tablets; Jantoven 1mg, 2mg, 2.5mg, 3mg, 4mg, 5mg, 6mg, 7.5mg, 10mg; and oxybutynin 5mg.
