Covidien is voluntarily recalling a single lot of ReliOn insulin syringes due to possible mislabeling. During the packaging process some U-40 syringes were mixed with syringes labeled for use with U-100 insulin. The use of these syringes may lead to patients receiving 2.5 times the intended dose of insulin, which may cause serious adverse events such as hypoglycemia and even fatality.
The ReliOn syringes in this recall were sold at Wal-Mart and Sam’s Clubs from August 1, 2008 until October 8, 2008.
