Indications for TOFRANIL

Childhood enuresis.

Children's dosing for TOFRANIL

<6 years: not recommended. ≥6 years: initially 25 mg daily 1 hour before bedtime; after 1 week, increase to 50 mg for children 6–12 years, up to 75 mg for those >12 years. Early night bedwetters: give 25 mg in afternoon and repeat at bedtime. Max 2.5 mg/kg per day. Attempt drug-free periods after gradual tapering of dose if drug response favorable.

Contraindications for TOFRANIL

During or within 14 days of MAOIs. Acute post-MI.

Precautions for TOFRANIL

Obstructive uropathies. Glaucoma. Cardiovascular disease. Monitor ECG. Epilepsy. Suicidal tendencies. Psychosis. Mania/hypomania. Hyperthyroidism. Diabetes. Renal or hepatic dysfunction. Pregnancy (Cat.C). Nursing mothers: not recommended.

Interactions for TOFRANIL

Hyperpyretic crisis, convulsions, death with MAOIs. Potentiates alcohol, sympathomimetics, CNS depressants. Potentiated by methylphenidate, CYP2D6 inhibitors; monitor plasma levels with cimetidine, SSRIs, phenothiazines, type 1C antiarrhythmics (eg, quinidine). Antagonized by barbiturates, carbamazepine, phenytoin. Paralytic ileus, hyperpyrexia with anticholinergics. Increased cardiovascular effects with methylphenidate. Blocks guanethidine, clonidine.

Adverse Reactions for TOFRANIL

Drowsiness, orthostatic hypotension, ECG changes, restlessness, insomnia, anticholinergic effects, CNS overstimulation, extrapyramidal symptoms, nausea, fatigue, rash, headache, changes in blood sugar, photosensitivity, edema, bone marrow depression (discontinue if occurs), jaundice, urinary frequency.
