Labeling Mix-Up Prompts Recall of Insulin Syringes

BD has issued a recall for the BD Insulin Syringes with the BD Ultra-Fine needle ½mL 12.7mm 30G, Cat (Ref) 328466 with Lot# 6291768.

The recall is being conducted because the polybags in this lot are incorrectly labeled as BD Ultra-Fine needle ½mL 8mm 31G, Cat (Ref) 328468. Patients who use a 12.7mm needle for injection, when an 8mm needle was intended, may run the risk of inadvertent intramuscular injection, which may lead to unanticipated hypoglycemia. No adverse events have been reported at this time. 

The shelf carton and case carton are correctly labeled as BD Insulin Syringes with the BD Ultra-Fine needle ½mL 12.7mm 30G. The affected lot was distributed between March 3, 2017 and April 24, 2017.

BD will provide a return kit and a voucher to customers affected by this recall. The voucher can be redeemed at a retail pharmacy for product replacement at no charge.
