Chronic open-angle glaucoma. Subacute or chronic angle-closure glaucoma after iridectomy or where surgery is refused or contraindicated. Certain non-uveitic secondary types of glaucoma, esp. glaucoma following cataract surgery.
Early chronic simple glaucoma: 0.03% soln instilled twice daily before bedtime and in the morning. Advanced chronic simple glaucoma and glaucoma secondary to cataract surgery: 0.03% soln instilled twice daily; if being transferred due to unsatisfactory control with pilocarpine, carbachol, epinephrine, etc, use higher strength soln. May be used concomitantly with epinephrine, a carbonic anhydrase inhibitor, or both. Apply finger pressure to inner canthus for 1–2 minutes after instillation.

Active uveal inflammation. Angle-closure glaucoma.
Discontinue or withhold if cardiac irregularities, salivation, urinary incontinence, diarrhea, profuse sweating, muscle weakness, or respiratory difficulties occur. Vagotonia. Bronchial asthma. Spastic GI disturbances. Peptic ulcer. Pronounced bradycardia. Hypotension. Recent MI. Epilepsy. Parkinsonism. Ophthalmic surgery. History of rentinal detachment. Quiescent uveitis. Myasthenia gravis. Gonioscopy recommended prior to initiating therapy. Pregnancy (Cat.C). Nursing mothers: not recommended.
Potentiates other cholinesterase inhibitors (eg, succinylcholine, organophosphate and carbamate insecticides).
Stinging, burning, lacrimation, lid muscle twitching, conjunctival and ciliary redness, browache, induced myopia, visual blurring, activation of latent iritis, uveitis, iris cysts (esp. in children), conjunctival thickening, obstruction of nasolacrimal canals, lens opacities, retinal detachment, paradoxical increase in intraocular pressure, cardiac irregularities.

Diagnosis: 1 drop of 0.125% soln instilled once daily in both eyes at bedtime for a period of 2 or 3 weeks. Treatment: 0.125% soln instilled once every other day or 0.06% soln instilled once daily. May gradually lower dose as treatment progresses. Max: 0.125% soln once daily.
