A new conceptual model consisting of ten building blocks can help practices become high-performing, patient-centered medical homes. Researchers from the University of California, San Francisco, Center for Excellence in Primary Care, Department of Family and Community Medicine used personal experience, site visits, and literature reviews to discover the elements that set the well-managed practices apart from the rest. These include:

  • Engaged leadership
  • Data-driven improvement (using health IT)
  • Empanelment (improving continuity of care)
  • Team-based care (clinicians and nonclinicians working side-by-side)
  • Strong patient-team partnership
  • Population management (creating teams to match patient needs)
  • Continuity of care
  • Prompt access to care
  • Comprehensiveness and care coordination 
  • The template of the future (going beyond the 15-minute appointment)
The model should not be interpreted as a "universal road map," but rather an "overview" for practice transformation.
